Will Your Next Mistake Be Fatal?

This sounds like a good read for anyone whose job includes any form of management. Written by Robert Mittelstaedt, who was a consultant for the NRC after the 3-Mile Island Nuclear accident, the book takes an investigative approach to learning from and preventing organizational disasters through a chain of related mistakes. Many large disasters (3-Mile Island, the shuttle Columbia, and the fall of Enron, for example) are the results of a series of mistakes rather than a single large error.
